The Wellness Coordinator develops an environment of health and wellness for Independent Living Residents.  The Wellness Coordinator assists the IL leadership team in the proactive identification of residents deemed at risk and provides support and assistance and referral to external agencies to ensure continued successful living in IL. POSITION SUMMARY The Hospice Case Manager is responsible for the provision of all patient care services assigned to them and develops the plan of care in accordance with the attending physician and the Interdisciplinary Team.
