  • Responsibilities

    The Overlook is looking for an experienced MDS Nurse to join our clinical team. The primary responsibility of this position is to conduct resident assessment and care plans.

    • Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies
    • Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care
    • Assist Unit Manager in creating care plans and ensuring that all personnel involved in providing care to the resident are aware of the resident’s care plan
    • Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment
    • Oversee weekly Medicare meeting
    • Submit MDS’s weekly to the Center for Medicare and Medicaid Services (CMS)
    • Participate in functions involving discharge plans as may be necessary.
    • Conduct a triple check at the end of the month and upload files to the business office

    Qualifications

    • Current licensure for Registered Nurse (RN) in MA, with a minimum of 2 years MDS experience ideally in a long-term care setting
    • Knowledgeable of the MDS process and time frames
    • PPS experience preferred
    • Proficiency with Medicare certifications and Medicare regulations as related to skilled nursing
    • Resident Assessment Coordinator (RAC-CT) certification preferred
