Responsibilities

The Wellness Coordinator develops an environment of health and wellness for Independent Living Residents.  The Wellness Coordinator assists the IL leadership team in the proactive identification of residents deemed at risk and provides support and assistance and referral to external agencies to ensure continued successful living in IL. This person will be responsible for identifying residents who need to transition to higher level of care and coordinates a successful transition. 

 

  • Facilitates Resident Transition Committee weekly meetings and maintains current documentation
  • Provides case management services for residents identified at risk or for residents who have transferred to an acute care setting. 
  • Monitors and facilitates transition for current residents to appropriate service levels and higher levels of care
  • Provides oversight for the Independent Living Community Health and Wellness Program
  • Reviews security incident reports that impact the health and safety of residents and follows up with identified residents
  • Implements health clinics and wellness presentations
  • Participates in Masonic Health Center discharge planning meetings for returning IL residents
  • Educates new residents and current residents to the nature and purpose of the wellness program and its translation into programs that support successful aging.
  • Identifies residents with memory loss, makes appropriate referral to transition team; collaborates with others in the development of programs that enable residents to remain engaged in community life
  • Participates in new resident orientation meetings (individual and group presentations)
  • Schedules health assessments for prospective residents and provides timely recommendations to sales team and Executive Director.  
  • Develops referral network for available programs and services and assists residents in accessing such programs and services in the external community
  • Participates in plan of care/discharging planning meetings for IL residents in Overlook Health Center/or other healthcare facilities to ensure a smooth transition back to IL

Qualifications

  • A current RN/LPN license for the State of MA required.
  • Minimum of three years related experience and training required, senior living preferred
  • Strong clinical assessment skills
